Never dealt with OSHA but I bet their inspections aren't too much different than MSHA. It's like anything human inspection based in that if they want to find something they'll find it. We had different inspections within a week of each other. First one we came out relatively well, second one not so much. Also, they're individual fines are also not anywhere close to the maximum. Although a few grand for a fine is probably a decent dent for a small, mom and pop shop.
