I went and test drove a 06 silverado 1500 with 5.3 and it sounds different at idle versus my 2004 Silverado 1500 5.3. Did they change anything on newer motors?

The injectors sounded different. Maybe because it was really cold outside.

Just 'cause its a newer model doesn't make it better. Depends on how it was cared for. Those injectors could be dirty and need a cleaning. A rough idle would be an indicator.

My 06 has had a lot of valve train noise and piston slap when it's cold. After everything has warmed up thoroughly it pretty much goes away. Never has used oil since the day I got it. There is ZERO wear in the cylinder walls at 125K miles. Not sure how the bearings look but I would assume they would look good too.

How many miles on the truck you're looking at?

Yes, if it is stumbling while idling, beware.

No run smooth just sounded different. Not knocking or anything. Truck has 110000 miles on it.

Some of the 05+ trucks were aluminum block...my 05 had the aluminum block and it sounded different than the 04 work truck with the iron.

That could be the difference. The 06 should have electric fans, so I would think the engine would sound noisier? Because all you are hearing is engine, no fan noise masking it?
